UPS Application for Enrollment

Admissions Procedure 

Submit the completed application online here or print it out here and mail it in with the following.

Mail in the following:

1. $25.00 nonrefundable application fee payable to “UrbanPromise Wilmington”
2. Copy of child’s birth certificate.
3. Copy of most recent income tax return, last two pay stubs or W-2 Form (if you wish to be considered for reduced tuition.) Does not apply for Pre-K
4. Current record of medical inoculations and completed physical examination document.
5. Current or final report card.
6. Discipline Records 
7. Copy of IEP or 504 Plan, if applicable. 
8. Court ordered custody or guardianship documents, if applicable.

To proceed with the acceptance process, all documents and fee(s) must be received by the school.   

Completing this initial process does not guarantee acceptance to UrbanPromise School
